Yes. Most nationalities, including US, UK, and EU citizens, need a visa, and it covers both the mainland and Zanzibar, as Zanzibar is part of Tanzania. Apply for the Tanzania eVisa online at least 3 to 4 weeks before travel: $50 for most nationalities, $100 for US citizens (multiple-entry, required regardless of trip length).
